| There are two ways you can register. 1. Register online now. |

2. You can register by using the registration form in your copy of the Registration and Main Announcement brochure. OR Print your own copy of the VALA2004 Registration Form. This form is published over the web in PDF (portable document format). To view a PDF file you will require a copy of the Adobe Acrobat Reader (a free product) installed on your computer. Click on the Acrobat Reader logo at the side to download a copy of Acrobat Reader.
Complete the registration form and
forward it by mail or fax to the address as noted, with your
payment. |

| Cancellation Policy: Registration Fees |

| Cancellations received in writing at the Conference Office thirty (30) days prior to the conference commencement date will receive a refund less a handling fee. No refund will be given after this date; however, an alternative delegate name may be submitted. All cancellations and substitutions must be made in writing to the Conference Office. | ||
